Best advice we ever got was that there comes a point where you need to breath or risk tearing the perineum (sp?). So our instructor taught me how to get in my wife's face and tell her "Breathe or tear" and then she had a ceaserean.

We just had a baby (boy) a month ago so I totally sympathize. All I can say is:
1) It's the best thing in the world,
2) Buy a sling. Trust me. It's the only way to carry a baby and be able to type at the same time, and
3) Remember #1 when it's 3am, you've spent the last bleary-eyed hour pacing around trying to put her to sleep (with no success), you feel a growing warmth in the diaper region, and then she starts crying. Did I mention it's the best thing in the world?
