It seems entirely plausible to me that he suffers from a medical condition which distorts his appearance. This is probably a self-selected "normal" picture. It's extremely rude to point and laugh at people with disfigurements or disabilities.

Man, if somebody posted a bad picture of me on the Internet and a bunch of people mocked it, I'd be like, "Heh, yeah, I do look like someone beat on me with an ugly stick there, don't I?" If someone posted a bad picture of me on the Internet and a bunch of people ASSUMED I WAS SOMEHOW DISFIGURED OR DISEASE-STRICKEN, then I'd be pissed off.
