What a wild coincidence. Just a few hours ago I finished reading The Secret of Ventriloquism, Padgett's collection in which this story appears. If you enjoyed "Twenty Simple Steps," I urge you to read the whole book, as the stories connect and intertwine in interesting and illuminating ways.

lapsed ventriloquist

That sounds like some reconstructive surgery, or at least physical therapy, is in order.

“Look at your body—
A painted puppet, a poor toy
Of jointed parts ready to collapse,
A diseased and suffering thing
With a head full of false imaginings.”

- Dhammapada, also in the intro to Ligotti's Conspiracy Against the Human Race. Ligotti really had a heck of a Puppets-n'-Clowns streak for a while there.
