The worst aviation disaster in history took place on the island of Tenerife when a KLM Boeing 747 attempting to take off slammed into a Pan Am Boeing 747 during heavy fog. "A memorial was dedicated at Tenerife airport on March 27, 2007. The 30th anniversary marks the first time that Dutch and American next of kin will join in a ceremony."

Seconds from Disaster (National Geographic Channel) did a great episode on this. I caught a rerun a month or so ago. It illustrated very well just how many things went wrong to allow this disaster to happen.
