HIV, like a computer virus, spreads mainly through poor social engineering (ie, the relative powerlessness of women, lack of public discourse on sexual topics, religous inflexibility, etc).
Correcting these problems will entail the education of the people, and there is nothing more dangerous to a repressive society than an educated populace.
